GDPR
My GDPR agreement sets out how I collect, store and process your personal data. I take all reasonable and necessary precautions to ensure that your data is kept secure and in accordance with GDPR regulations.
How I gather your personal information:
Information that you provide via email, phone, website or online therapist directories (BACP, Counselling Directory, Psychology Today)
Counselling contract agreement
Brief session notes
Where I store your data:
Contact details are kept securely under a client code
Session notes are anonymised and kept separately
Computerised data is password protected
I use your contact details to provide you with information on my service and to notify you of any changes (admin/availability).
I take brief session notes to ensure good practice.
How long I keep your data for:
Email and text communication is deleted when no longer necessary
Contact details are deleted after therapy has ended
I will retain your session notes for 5 years after therapy has finished and then they will be destroyed
You are entitled to see, amend or delete the personal information that I hold.
In the event of a data breach:
If the data breach poses any kind of risk, I have a legal obligation to report it to you and the Information Commisioners Office (ICO) within 72 hours.
Disclosure of personal information:
In the event of my incapacity or death during our work together, your contact information will be disclosed to the clinical executioner of my Professional Will, so that they may notify you
If I am under a duty to disclose your personal data in order to comply with any legal obligation, for example; safeguarding, subpoenas or terrorism.
Your use of the services provided by Marie Claridge Counselling constitutes your acceptance of this agreement and consent to the use of your personal information, as outlined above. You have the right to withdraw consent at any time.
Covid Agreement (No longer in effect)
During the Covid pandemic, I will be following government advice on how to stay safe. Changes in that advice may result in temporary changes to my practice.
I have introduced a variety of measures to provide a safe environment:
Regular lateral flow test
Double vaccinated, 2x booster
Use and provide hand sanitiser
Counselling chairs are spaced apart to allow for social distancing
Well ventilated room
All surfaces disinfected
Can wear a visor at clients request
Telephone sessions available
What I require from you:
Be on time (no more than a few minutes early)
Sanitise hands
Keep your belongings with you
Bring your own drink
Take rubbish with you
Please note - If you are considered clinically vulnerable, it is advised that you do not attend face to face sessions.
Facilities:
The facilities that I use provide a waiting area, toilets and cafe for visitors. These are regulated and maintained by them.
Symptoms:
In the event that either you or I develop symptoms, you should not attend the session in person. Where possible, the session may be changed to a remote session. In this situation, I will try and give you as much notice as possible and I ask that you do the same.
Replies may be slower during weekends and bank holidays.
